Approval is hereby granted for the renovation ofa sewage disposal system located at 71
Beckwith. R.d., in the Town of Montville.

INSTRUCTIONS TO THE INSTALLER
1) The Uncas Health District is to be contacted before any change is made in the system location,
size or design.
2) All construction must be under the direct supervision of an installer licensed under section 20-
341 of the Connecticut General Statutes.
3) The installer is responsible for verifying the locations of property lines, water lines, wells and
buried utility lines prior to installation.
SPECWC INSTRUCTIONS:
Using the existing 1,0140 gallon septic tank and 6'x 6 dry well, install two (2)
additional ST x 61 dry wells per submitted plan for a total of 570 square feet
of leaching area-
