Mr. Lomonte:
In order to complete your application the following material is needed:
1. The disturbance area needs to be filled out on page 2 of the
application for the upland review area
- This is the new disturbance area in square feet
2. A narrative needs to be provided that has greater detail about
what you are doing , why you are doing it, and how you will be doing it.
3. The application has to be submitted formally. I can except the
one provided until Thursday and then I will need a clean copy that is
full size.
4. Map should show area of impact and the erosion and sediment
controls that will be used during construction
- The length and width of area and location of silt fence or hay
bales
Even though your application was to be in the office on Wednesday of
last week I will put you on the agenda because we discussed this a few
weeks ago. In the future though applications are due in the office the
Wednesday the week before the meeting. So the meeting will be Thursday
night the 20th at 7:00 pm. The Commission can go either way and accept
your application and do nothing that night or since it is a modification
they may approve it. However for the latter you would have to have the
answers to any questions that may arise and all the paperwork and maps
must be done.

Hello Colleen;
Enclosed, kindly, you will find the application for additional work that
we need to correct erosion problem at our property an dspecifically
along the left corner facing the lake.
Please note that there is already a retaining wall along this area, and
a permit was taken in the past by our neighbhors located at 53 Lake
Drive, i. e. PauletteRydzy. As you also can see from the A-2 Survey Map
enclosed, there is a wall already in this area but we need to correct an
existing erosion problem along this area, as noted.
Please advise. Thank you for your anticipated courtesy and cooperation.
