problems that ere encountered. When we spoke you told me that you would go to the town hall of
Montvilie,Ct. d apply for the building permit that was required BEFORE the start of the job. Since then I
have heard no ing from you concerning any of the complaints that I have. Obviously you think that this
matter is unim rtant to me;but I assure you that I am treating this with the utmost attention. I would

Dear Jim & Chris, and Whom it may concern,
Jim, we enjoyed meeting with you. We were called after questioned regarding a liner installation that had
taken place at the above ddress. During our conversation it seemed that some things that were done by Jade
Enterprises was question ble. Jim and Chris asked if we were willing to come out and evaluate the work that
was done. Following are a few pictures to better explain what was found.
The picture to the left is of the chimney top where
the liner was installed. As you can see in this pic-
ture, the mesh of the cap, and the top of the chim-
ney are loaded with creosote. This was supposedly
. n r addressed in the invoice given to the Customer at
no charge. You will also notice the liner is very

close, too close only 3" approximately, to the cap
I lid. This along with downsizing the flue as was
4 - done, creates a serious health concern for the occu-
pants of the home, involving carbon monoxide.

The picture directly below shows where the top
support is not in it's proper position, sealing the
top of the flue. You can also see another sample of
y the creosote left on top of the chimney, which was
- • supposedly cleaned.

This picture to the left is looking down the liner that
was installed for the boiler flue. I could find no infor-
mation on the liner without taking it apart, as to the
- - manufacturer, and the Homeowner had no information
or warranty. Without this information I cannot be sure
f.' ■ that a 321, 316, or 316TI alloy was used, which is nor-
mally required for boilers. There was scrap liner left

`r w on the Homeowners property, which feels like alumi-
num, unacceptable for an oil boiler liner. Also, after
seeing how the liner is "puckered" in where screws
mar= _
x were installed, it appears the liner was probably in-
stalled upside down. Any liner we have installed is
made to drip the condensates & acids down. These
' l areas will catch and hold those condensates & acids as
installed, probably causing premature liner failure.
Also most liner manufacturers require rivets be used
because the screws are not normally manufactured in
316 alloy, as the rivets are.
While on the roof, checking a liner out, I inspected the -
fireplace flue. It seems in Ja e Enterprises invoice that this
was supposedly addressed. Fight after "Instructions" after
the Customer's address, it states "F/P & Oil $75.00 NC". _ - x
One would assume this means both would be addressed. As
you can see in the picture to he right, which is the fireplace - • - ,
flue, it is very dirty. Also of most concern, the 1 st flue - - j r
down is cracked badly. The ize of the cracks would indi
cate to most professionals that: #1. This flue requires further
inspection, which can only b done by cleaning it. #2. This s
should have been noted upo inspection/evaluation. 93. Y„
The Customer should have b en told not to use this fire-
place & flue because of a sa ty concern (see NFPA guide r x
line on first page). 44. With a crack this size it is more than

likely more flues are cracked below.

The 2 pictures below are loo ing up into the fireplace and flue through the damper. You can see in these pictures that this flue con-
tains a lot of dangerous creo ote. This does not seem to have been addressed at all, putting this Homeowner at serious risk.

The picture to the left is the boiler connection to the
' .01 liner. I was concerned when I saw the tape on the
- joints of the connector pipes. We took the connec-
tion apart where it goes into the chimney, with Jim
Philip's permission. We did find 2 screws connect-
ing the pipes to the piece in the wall. Three screws
are required in each joint minimum.

snout, after the connector pipes were removed. The
Contractor put a galvanized adapter inside the wall
` - (concrete foundation), which is mortared around.
Furthermore the liner is only a 5.5" id, the boiler
connector is a 7" id. Most Building Inspectors re-
quire us to extend all the way out from the liner to
just beyond the face of the wall with stainless steel.
? This is due to they want the galvanized, which can
1 I~ corrode, only out side the foundation, where it can be
watched & replaced as needed.
Finally, this last picture belo is looking up, into the bottom' N
of the liner, from the cleanout door. My concerns are: #1'

In Summary: The items that were done, or not done raise many
y questions.
L What was really cleaned, if anything even at no charge?
2. Top support not positioned correctly.
a: 7 3. Is the liner installed upside down, as it appears?
4. Why wasn't the cracked fireplace flue brought to the atten-
tion of the Homeowner?
5. Why wasn't the "stainless" connection extended out of the
masonry?
6. Why was the boiler connector reduced in size?
7. How "safe" is this Homeowner against exhaust fumes
(carbon monoxide) entering the home?
Also:
1. CT Home Improvement Contractor licensing requires in
contracts over $300.00 the Customer have a 3 day right of
rescission, where is that?
2. We could find no reference to the CT License number be-
ing valid.
At your request we will be glad tc, offer a proposal for proper
repairs needed to this chimney.
