Kirby, R.S., Chief Environmental sanitarian Project Name: 9R Burlake LLC Project Location: 120 Gay Hill Road Comments: Plan revised 7-27-09 The septic system on the Rodigou property, 28 Swanty Johnson Road must be located. Or, the wells on lots 6 & 7 must be located 75 feet from the property line. The septic system on the St. Pierre property, 16 Swanty Johnson Road must be located. Or, the well on lot 3 must be located 75 feet from the property line. As noted, the sewer mains, sewer laterals, and force mains must be approved pipe from Tables 2-A, 2-B and 2-D of the CT Public Health Code’s Technical Standards. Many of the wells are located the minimum of 25 feet from the sewer laterals. The sewer laterals are to be staked to ensure that the required separating distance to the wells is maintained. Lot 30 -The well is less than the required 25 feet from the roof leader. Lots 32, 33, & 34 – Clarify what the UGP along the road is. If it is an underground drain, the required 25 feet separating distance from the wells on these lots must be maintained. A maintenance plan for the ponds should be developed to prevent any stagnation of the water that may result in a mosquito nuisance. Electronic Signature: Michael J. Kirby       Page 2 of 2
