RE: Monahan Drive Resubdivision

Dear Liz:
We have reviewed the plans and supporting documents submitted for the proposed
resubdivision of land beyond the existing terminus of Monahan Drive. We have the
following comments:
1. The as-builts for Monahan Drive differ from the submittal. The drawings of
the existing conditions must be updated.
2. The plans must show the work and materials as conforming to CT DOT Form
818 in lieu of the outdated Form 816 noted on the plans.
3. The data for the test holes and monitor wells must be shown on the plans.
4. The direct stormwater discharge shown on Lot 12 must be redesigned to
conform to MS-4 requirements.
5. The proposed detention pond is described to be maintained by a home owners
association. The existing detention pond on Gay Hill Road is in need of
maintenance. Is there an association?
6. Note 23 on the front sheet describes inspection fees; the Phase 2 fee was
determined some 13 years ago and must be updated.
7. Note 25 on the front sheet must be updated to present day costs.
8. Snow storage areas at the turnaround must be shown in accordance with present
road standards.
9. The calculations for the size and the installation of the cross culvert on Lot 7
must be provided.

10. The inlet for the drainage on Lot 7 must be revised to a DG endwall or flared
end unit in lieu of the C-L type catch basin.
11. The calculations for the sizing of the level spreaders must be provided.
12. The access gate to Swanty Johnson Road must be supported by steel end poles
in lieu of timber poles.
13. The details must be updated in accordance with current standards, especially
regarding pavement sections.
14. The proposed connection to the existing pavement on Monahan Dr. must be
detailed on the plans.
15. The erosion and sediment plans must be detailed for the roadway construction.
16. The driveway details must be added to the plans.
17. The grades within the turnaround on Beacon Road are too flat.
18. Flared end units shall be added to all outlets.
19. Connection of foundation drains and sump pumps must be updated in
conformance with 90I of the roadway standards.
20. Street lights exist on the exiting portion of Monahan Drive and must be shown
on the proposed extension of the roadway.
21. The proposed grading for pond #3 does not meet current standards.
22. The storm intensity data must be updated in the drainage study.
23. The E&S Control Bond estimate for the roadway work must be submitted for
review.
24. The construction bond for the public improvements must be submitted for
review.
