Project Name: Jeremiah Kaplan Project Location: 635 Fire Street Comments:
1. Additional test holes must be dug in the primary and reserve areas.
2. As designed there is not any down gradient area for MLSS calculations. The average restrictive layer is 21.7". Therefore the MLSS is 72 x 1.5 x 1.2 = 129.6'
3. The percolation test is to be located on the plan. If it is not in the primary area
another one must be done in the primary area. 4. The required separating distance between the primary and reserve areas must be shown on the plan.
5. The existing well on the adjacent property must be shown on the plan. 6. North is to be indicated on the plan.
