DESIGN IS FOR THREE BEDROOMS WITH NO GARBAGE GRINDER
AND NO TUBS OF 100-200 GALLONS WITH A PERC RATE OF
LESS THAN 1.0 MIN./IN. SYSTEM AREA REQUIRED IS 495 S.F.
AND IS SUPPLIED AS FOLLOWS:
NO RESTRICTIVE LAYERS PER TESTING.
MLSS IS NOT APPLICABLE.

3. THERE ARE NO WELLS WITHIN 75' OF THIS SYSTEM.
4. A CONDITION OF THIS DESIGN IS THAT THE SUBSURFACE SEWAGE
DISPOSAL SYSTEM (SSDS) SHALL BE FIELD STAKED BY A LICENSED
SURVEYOR PRIOR TO CONSTRUCTION.
5. SITE INVESTIGATION AND SUBSEQUENT FOUNDATION ELEVATIONS
ARE FOR THE SOLE PURPOSE OF SANITARY SEPTIC DESIGN. ANY
FOOTING DRAINS OR SUMP OUTLETS DEPICTED ARE FOR THE SOLE
PURPOSE OF ENSURING THAT NO CONFLICT EXISTS BETWEEN SUCH
DRAINS AND THE SSDS. IT SHALL BE THE SOLE RESPONSIBILITY
OF THE OWNER AND HIS OR HER CONTRACTOR TO DETERMINE THE
ADEQUACY OF ANY SUCH ELEVATIONS AND DRAINS INTENDED
TO PROTECT THE BASEMENT FROM WATER INCURSION AND/OR
SEEPAGE.
INSTALLER NOTES:
1. THE LICENSED INSTALLER IS RESPONSIBLE FOR INSTALLING THE
SUBSURFACE SEWAGE DISPOSAL SYSTEM (SSDS) IN ACCORDANCE
WITH THE APPROVED PLANS. IN ADDITION, PER HEALTH CODE
REQUIREMENTS THE LICENSED INSTALLER SHALL BE ON-SITE
DURING CONSTRUCTION OF THE SYSTEM.
2. ALL TOPSOIL, STUMPS, AND BOULDERS MUST BE REMOVED FROM
THE AREA OF THE SSDS. SILT FENCE SHALL BE INSTALLED PRIOR
TO ANY SSDS, DRIVEWAY, HOUSE CONSTRUCTION, OR STUMP
REMOVAL.
3. TOWN SANITARIAN OR HEALTH DEPARTMENT MUST BE NOTIFIED
BEFORE ANY PORTION OF THE SSDS IS CONSTRUCTED AND PRIOR
TO BACKFILL OF ANY PORTIONS.
4. THE INSTALLER SHALL BE RESPONSIBLE FOR AND ADEQUATELY
COMPACT ANY FILL THAT IS REQUIRED OR USED FOR OR IN
CONJUNCTION WITH THE SSDS. SAID FILL SHALL BE PLACED IN
6"-8" LIFTS AND SHALL BE MECHANICALLY COMPACTED.
5. THERE SHALL BE NO CUTS WITHIN 50' OF THE DOWNHILL SIDE OF
THE SYSTEM OR WITHIN 25' OF THE SIDES OF THE SYSTEM. THERE
SHALL BE NO STUMPS OR BOULDERS BURIED WITHIN 50' OF THE
DOWNHILL SIDE OF THE SYSTEM. THERE SHALL BE NO FOOTING
DRAINS OR STONE PLACED AROUND OR UNDER ANY PROPOSED
STRUCTURE WITHIN 50' (DOWN GRADIENT) OF THE SSDS.
6. SHOULD ANY SIGNIFICANT VARIATIONS FROM TEST HOLE DATA
SHOWN (LEDGE, WATER, MOTTLING, SOIL TYPE) BE DISCOVERED BY
THE CONTRACTOR, THE ENGINEER SHALL BE NOTIFIED PRIOR TO
CONSTRUCTION OF THE SSDS.
7. IN GENERAL, ALL CONSTRUCTION PRACTICES, AS PER THE STATE
OF CONNECTICUT DEPARTMENT OF HEALTH SERVICES GUIDELINES,
SHALL BE ADHERED TO. THE FOLLOWING OUTLINES SOME OF THE
MAJOR PROCEDURES, BUT DO NOT RELIEVE THE CONTRACTOR
FROM FULL COMPLIANCE DUE TO THEIR LACK OF MENTION BELOW:
A. ALL TOPSOIL, STUMPS, AND BOULDERS SHALL BE REMOVED
FROM THE AREA OF THE SYSTEM AND SYSTEM FILL. NO
MACHINERY OR VEHICLES SHALL BE ALLOWED ON THE
EXPOSED NATIVE SOIL. STRIPPING SHOULD NOT TAKE PLACE
WITHIN 48 HOURS OF A RAIN STORM OR IF THERE -r.;
