Comments:
1.) An additional test hole is required at the north end of the 15 foot Mantis field. The
additional test hole and test hole 1 is to be dug again to a minimum depth of 8.5 feet or
the system is to be raised 2.5'.
2.) Approval of Well Exception from CT State SSDS reducing the separation distance
requirement between the existing on site well and new SSDS structures.
3.) Approval from local wetlands commission authorizing repair work within wetland
delineation.

I do have the plan and will be reviewing it today when I get back to the office. Risk assessment wise, if the system has
discharged to the surface once and there is a risk of doing so again I would say it needs to be repaired ASAP because the
proximity to an open water course.That of course doesn't do anything in regards to the state approval, it depends on
what they will need and their current work load.
