1 . Contours from field survey by David Coonrod, L.S.
2. Proposed house is a four bedroom house. Basis of design
is for four bedrooms with a design perc rate of 1.0 -
10.0 min./in. System area required is 660 s.f. and is
supplied as follows:

MLSS Required = HF(26) x PF(1.2) x FF(2.0) = 62'
MLSS Supplied = 120'
A 100$ reserve area has been supplied.
3. All topsoil, stumps, and boulders must be removed from
the area of the system.
4. Town sanitarian must_ be notified before any pipes or
appurtenances are backfilled.
5. There are no wells within 75' of this system.
6. Gravel fill shall be used to replace all topsoil and
subsoil as required by the Toon Sanitarian. Gravel
shall be placed in 6"-8" lifts and shall be
mechanically compacted for both the primary and reserve
areas, as shown on this plan.
7. Gravel fill shall have a maximum of 5* fides passing a
Standard No. 200 sieve and a maximum of 109 passing a
No. 100 sieve. All gravel fill shall be approved by
the Town Sanitarian prior to deposition. Gravel fill
in place shall have a pert rate equal to or faster than
the design rate.
8. There shall be no cuts within 50' of the downhill side
of the system or within 25' of the sides of the system.
There shall be no stumps or boulders buried within 50'
of the downhill side of the system. There shall be no
footing drains or stone placed around or under any
proposed structure within 50' (down gradient) of the
system.
9. Should any significant variations from test hole data
shown (ledge, water, mottling, soil type) be discovered
by the contractor, the engineer shall be notified prior
to construction of the system.
10. System area and elevation shall be staked out by the
engineer or surveyor prior to construction.
11. In general, all construction practices, as per the
State of Connecticut Department of Health Services
guidelines, shall be adhered to. The following outlines
some of the major procedures, but do not relieve the
contractor from full compliance due to their lack of
mention below:
A. All topsoil, stumps, and boulders shall be
removed from the area of the system and
system fill. No machinery or vehicles shall
be allowed on the exposed native soil.

.� B. The earth shall be scarified prior to fill
- 116 placement. Gravel fill shall meet the
specifications of Note 7 and be placed in
accordance with Note 6.
C. No trucks or heavy machinery shall be allowed
in the stripped area until 12" of gravel fill
is in place.
12. Tank manhole risers may be required to within 12
inches of finished grade if top of septic tank is
greater than 12 inches below grade.
