THIS PROJECT INVOLVES THE DEVELOPMENT OF A 221,933± SF (5.09± ac) PRE-EXISTING REAR
LOT TRACT OF LAND TO INCLUDE A FIVE (5) BEDROOM RESIDENCE WITH ON-SITE SANITARY
SEWER AND WELL WATER. TOTAL LAND DISTURBANCE WILL BE LESS THAN 0.5 ACRES, WITH NO
FILLING OF ANY REGULATED WETLAND AREAS.
SILT FENCE WILL BE INSTALLED DOWN GRADIENT OF ALL CONSTRUCTION AREAS PRIOR TO
CONSTRUCTION. AN ANTI -TRACKING PAD SHALL BE INSTALLED PRIOR TO STRIPPING AND GRUBBING
OPERATIONS. TOPSOIL SHALL BE STOCKPILED ON SITE, SURROUNDED BY SEDIMENT FENCE AND
SEEDED WITH RYEGRASS DURING CONSTRUCTION.
IT IS THE INTENT OF THESE PLANS TO STABILIZE ALL DISTURBED AREAS AS SOON AS
PRACTICABLE AND ESTABLISH VEGETATION OR INSTALL THE PARTICULAR COVER PROVIDED. THE
CONTRACTOR SHALL ATTEMPT TO ANTICIPATE AND CORRECT ANY PROBLEM AREAS OR FAILURES OF
THE EROSION AND SEDIMENTATION SYSTEMS.
RESPONSIBILITY FOR THE INSTALLATION OF THE EROSION AND SEDIMENTATION PROTECTION AS
IDENTIFIED IN THIS NARRATIVE SHALL BELONG TO THE APPLICANT/OWNER. TRACY McNICHOL AT
(860) 442-9172 IS THE DESIGNATED ON-SITE AGENT RESPONSIBLE FOR ASSURING TO THE TOWN
THAT ACTUAL MEASURES TAKEN COMPLY WITH THE CERTIFIED PLAN. WHEN A DIFFERENT
INDIVIDUAL BECOMES THE DESIGNATED AGENT, THE TOWN MUST BE NOTIFIED AND ASSURED THAT
THE INDIVIDUAL IS ADEQUATELY QUALIFIED TO ACT AS AGENT.
ALL MEASURES SHALL BE INSPECTED ON A WEEKLY BASIS BY THE DESIGNATED ON-SITE AGENT.
THE TOWN STAFF WILL ALSO INSPECT THESE MEASURES AT ANY TIME THE STAFF DEEMS
NECESSARY. ANY FAILURE OF THE E & S MEASURES SHALL BE CORRECTED IMMEDIATELY AND
ANY EROSION OR SEDIMENTATION BEYOND THESE LIMITS SHALL BE REMOVED TO THE SATISFACTION
OF THE TOWN STAFF.
THE E & S MEASURES ON THE ENTIRE SITE SHALL REMAIN IN PLACE, INSPECTED BY THE
ON-SITE AGENT, AND MAINTAINED BY THE CONTRACTOR UNTIL THE ZONING ENFORCMENT OFFICER
IS SATISFIED THAT THE ASSOCIATED SITE HAS BEEN STABILIZED.
CONSTRUCTION NOTES:
1. ALL EXISTING UTILITIES ARE APPROXIMATE ONLY. IT IS THE RESPONSIBILITY OF THE
CONTRACTOR TO VERIFY ALL LOCATIONS PRIOR TO CONSTRUCTION. "CALL BEFORE YOU DIG" IS
TO BE NOTIFIED PRIOR TO ANY CONSTRUCTION. (800-922-4455).
2. THE CONTRACTOR IS RESPONSIBLE FOR OBTAINING ALL APPLICABLE CONSTRUCTION PERMITS,
PRIOR TO CONSTRUCTION, FROM THE MONTVILLE PUBLIC WORKS DEPARTMENT AND THE MONTV"LLE
ZONING ENFORCEMENT OFFICER ALONG WITH ANY OTHER APPLICABLE AGENCIES. ALL AGENCIES
SHALL BE NOTIFIED AT LEAST 48 HOURS PRIOR TO CONSTRUCTION.
3. THE CONTRACTOR IS RESPONSIBLE FOR VERIFYING ALL DISTANCES AND LOCATIONS AND
QUANTITIES AS DEPICTED ON THE APPROVED SITE PLAN. IF THERE ARE ANY DISCREPANCIES, THE
ENGINEER SHALL BE NOTIFIED PRIOR TO ANY CONSTRUCTION MODIFICATIONS.
4. THE E & S NARRATIVE CONTAINED IN THESE PLANS ARE CONSIDERED PART OF THE DESIGN
OF THIS SITE AND SHALL BE CONSULTED AS CONSTRUCTION DETAILS.
5. ALL EROSION AND SEDIMENTATION CONTROL MEASURES APPLICABLE TO A GIVEN AREA OF
CONSTRUCTION SHALL BE IN PLACE AND APPROVED BY THE TOWN STAFF PRIOR TO THE
COMMENCEMENT OF WORK IN THAT AREA.
6. ALL UPLAND AREAS OF SHALL BE FINISH GRADED WITH 4" OF TOPSOIL AND SEEDED AS
DELINEATED IN THE SEEDING GUIDELINES.

EVALUATE PROPOSED COVER MATERIAL
BEFORE SPREADING PROPOSED COVER MATERIAL OVER THE DESIGNATED AREA, OBTAIN A
REPRESENTATIVE SOIL SAMPLE AND SUBMIT TO A REPUTABLE SOIL TESTING LABORATORY FOP,
CHEMICAL AND PHYSICAL ANALYSIS. THIS PRELIMINARY TEST IS NECESSARY TO DETERMINE: T-iE
REQUIRED INORGANIC AND/OR ORGANIC AMENDMENTS THAT ARE NEEDED TO ASSIST IN
ESTABLISHING THE SEED MIXTURE IN AN ENVIRONMENTALLY AND ECONOMICALLY SOUND MANNR.
THE RESULTS WILL GIVE THE COVER MATERIAL CHARACTERISTICS SUCH AS PH AND FERTILIZN'ION
NEEDS. THESE RESULTS SHALL BE KEPT ON-SITE BY THE CONTRACTOR AND AVAILABLE FOF
REVIEW BY THE TOWN.
SEED BED PREPARATION
PROPOSED COVER MATERIAL SHOULD BE SPREAD EVENLY OVER THE SITE AREA IN A MINIMUM 4"
LIFT VIA BULLDOZER/BUCKET LOADER. USING THE INFORMATION FROM THE SOIL ANALYSIS,
CAREFULLY CALCULATE THE QUANTITIES OF LIMESTONE AND PRE -PLANT FERTILIZER NEEDED
PRIOR TO APPLYING. PRE -PLANT AMENDMENTS CAN BE APPLIED WITH A BROADCAST AND,/C'7
DROP SEEDER AND INCORPORATED WITH AN OFFSET DISK, YORK RAKE, AND/OR HAND RAKE.
AFTER INCORPORATING THE PRE -PLANT SOIL AMENDMENTS, THE SEED BED SHOULD BE SMOOTH
AND FIRM PRIOR TO SEEDING. THE FOLLOWING SEED MIXTURES SHALL BE USED AS NOTE=D;

SEED TIME AND METHOD
THE PREFERRED TIME FOR SEEDING THE COOL SEASON MIXTURE IS LATE SUMMER. SOIL ANG
AIR TEMPERATURES ARE IDEAL FOR SEED GERMINATION AND SEEDLING GROWTH. WEED
COMPETITION IS REDUCED BECAUSE SEEDS OF MANY WEED SPECIES GERMINATE EARLIER IN THE
GROWING SEASON. ADDITIONALLY, HERBICIDE USE IS GREATLY REDUCED. HOWEVER, SEEDING;
MAY BE DONE AT ANY OF THE ABOVE NOTED TIMES.
MULCHING
NEWLY SEEDED AREAS SHOULD BE MULCHED TO INSURE ADEQUATE MOISTURE FOR SUCCESSFOL
TURF ESTABLISHMENT AND TO PROTECT AGAINST SURFACE MOVEMENT OF SEDIMENT -BOUND
AGROCHEMICALS AND SOIL EROSION. APPLY GOOD QUALITY STRAW OR HAY AT A RATE OF 12
BALES/1000 SQ. FT. OTHER COMMERCIALLY AVAILABLE MULCHES CAN BE USED.

SEPTIC DESIGN NOTES
1. THE SEPTIC SYSTEM IS TO BE CONSTRUCTED ACCORDING TO THE CONNECTICUT PUBLIC HEALTH
CODE REGULATIONS AND TECHNICAL STANDARDS FOR SUBSURFACE SEWAGE DISPOSAL SYSTEMS.
2. THE SANITARY SYSTEM IS DESIGNED FOR A PROPOSED FIVE (5) BEDROOM HOUSE.
THE PERCOLATION TESTS YIELDED RATES OF 4 MINUTES PER INCH, A TATE OF LESS THAN 10.1 MIN/IN.':H
HAS BEEN USED FOR DESIGN PURPOSES. THIS REQUIRES 825 SF OF EFFECTIVE LEACHING AREA.
THE PROPOSED SEPTIC SYSTEM IS AS FOLLOWS: 1,500 GALLON SEPTIC TANK AND
2 ROWS CSD 70LF OF 48" x 18" LEACHING TRENCHES BY SERIAL DISTRIBUTION.
LEACHING AREA PROVIDED= 136' OF GALLEY x 6.2 SF/LF= 843.6 SF EFFECTIVE
4' OF STONE x 6.2 SF LF= 24.6 SF EFFECTIVE
TOTAL = 868.0 SF EFFECTIVE
A 100% RESERVE AREA IS PROVIDED.
MLSS = NEED NOT BE CONSIDERED FOR RESTRICTIVE LAYER > 60".
3. PROPOSED HOUSE SEWER SHALL BE CAST IRON PIPE OR PVC SCHEDULE 40 ASTM D 1785.
ALL OTHER SEWER PIPE FOR THE SEPTIC SYSTEM SHALL BE 4" PVC ASTM D3033 OR D3034 WITH
RUBBER COMPRESSION GASKET BELL AND SPIGOT OR APPROVED EQUAL.
4. THERE ARE NO WELLS WITHIN 75' OF THE SEPTIC SYSTEM.
5. THERE SHALL BE NO CUTS WITHIN 50' OF THE DOWNHILL SIDE OF THE SYSTEM OR WITHIN 25' OF
THE SIDES OF THE SYSTEM. THERE SHALL BE NO STUMPS OR BOULDERS BURIED WITHIN 50' OF THE
DOWNHILL SIDE OF THE SYSTEM.
6. SHOULD ANY SIGNIFICANT VARIATIONS FROM TEST HOLE DATA SHOWN (LEDGE, WATER, MOTTLING,
SOIL TYPE) BE DISCOVERED BY THE CONTRACTOR, THE ENGINEER SHALL BE NOTIFIED PRIOR TO
CONSTRUCTION OF THE SYSTEM.
7. ALL EXISTING UTILITIES ARE APPROXIMATE ONLY. IT IS THE CONTRACTOR'S RESPONSIBILITY TO
VERIFY ALL LOCATIONS PRIOR TO CONSTRUCTION. "CALL BEFORE YOU DIG" SHALL BE
