Yes No CO Approved ☐ ☒ Comments/Observations Resolution Date: 10/3/2016 10/03/16 – 12 pm On-site inspection with NDPU Mark Decker, Water & Wastewater Integrity Manager, Mark Gaudette, Utility Engineer, NDPU Consultant Paul Burgess, Commercial Construction Group, LLC/Generators On Demand, Swaney (Responsible Party), Swaney Consultant, Don Fortunato, C.S.S. & Town Consultant, Bob Russo, C.S.S. Paul Burgess states he is recommending to NDPU it enter the Voluntary Remediation Program through DEEP in order for it to obtain an official sign-off. Conducted a site walk with Burgess, Fortunato, Swaney & Russo (1:00pm). Observed post-excavation conditions. Fortunato confirmed areas excavated to remove contaminated soils are wetlands areas. Burgess & Fortunato recommend further excavation to remove contamination – still odor of diesel fuel in soil. Additional excavation work will be performed on Tuesday, 10/04/16. Walked site past excavated areas that were contaminated with diesel fuel, but not able to be excavated due to very wet soils & stream. Booms and absorbent pads were observed at intervals along the stream. Sandbag dam had been constructed at area of stream prior to Stony Brook. Water clean – no sheen or oil detected past this point and additional booms had been installed at the intersection of the stream with Stony Brook. However, absorbent pads need to be place in stream as pockets of oil were detected up to the area of the dam. Swaney will install additional absorbent pads already on site. Reviewed next steps with Fortunato, Swaney & Russo. Additional absorbent pads to be placed in pockets of stream from excavated areas to dam area by Swaney. Additional excavation work to further remove contaminated soils will be performed 10/4/16 and areas to be seeded with wetlands seed mix to temporarily stabilize soil. A plan for wetlands restoration shall be prepared and submitted to the Montville Wetlands Commission. Open soils remediation/wetlands restoration project.
