You are hereby ordered to discontinue the violation at the above referenced property per
Section R113.1 of the 2005 Residential Code as adopted as the Connecticut State Building Code
You must STOP WORK as per Section R114.0 of the 2005 Residential Code as adopted as the
Connecticut State Building Code and you must submit to the Building Department a plan of compliance within ten
(10) calendar days from the date of receipt of this notice in order to avoid possible legal action.

Conditions
• Due to a fire in the boiler at the above listed •
address the boiler has been shut down and is not
to be restarted until the unit the boiler has been
inspected, repaired and certified by a licensed
8/31/07 DJ plumbing and heating contractor.
Heating system • There are five or more circulation pumps on the
heating system.One of the pumps has been
improperly connected to an electric hot water
heater causing a cross contamination problem
with the potable on site water supply.
• The following is a notice of an unsafe condition •
that exists in the electric system of the above
listed residence. Upon inspection of the boiler it is
noted that there are a number of unsafe and

Electric system must be contracted to repair and certified that the
required electrical deficiencies have been
repaired. Remove all unsafe and improper wire
connections. Install missing switch,outlet and
cover plate to J-boxes.
• Upon the separation of the heating system with •

Water System recommended that the water be tested prior to use
of the water supply.
NOTE: ALL OF THE ABOVE REQUIRED REPAIRS REQUIRE THE APPROPREATE BUILDING,
ELECTRICAL AND MECHANICAL PERMITS.
