INSPECTION Not Approved Approval Date: Deficiencies Special Date Conditions • Due to a fire in the boiler at the above listed • address the boiler has been shut down and is not to be restarted until the unit the boiler has been inspected, repaired and certified by a licensed 8/31/07 DJ plumbing and heating contractor. Heating system • There are five or more circulation pumps on the heating system.One of the pumps has been improperly connected to an electric hot water heater causing a cross contamination problem with the potable on site water supply. • The following is a notice of an unsafe condition • that exists in the electric system of the above listed residence. Upon inspection of the boiler it is noted that there are a number of unsafe and 8/31/07 DJ hazardous conditions exists.A licensed electrician Electric system must be contracted to repair and certified that the required electrical deficiencies have been repaired. Remove all unsafe and improper wire connections. Install missing switch,outlet and cover plate to J-boxes. • Upon the separation of the heating system with • 8/31/07 DJ the potable drinking water supply system it is Water System recommended that the water be tested prior to use of the water supply.
