The chimney liners covered in this category are inte ed for installation, (1)as : ternate lining systems
for fire clay flue linings (as described in ASTM C31 ) or(2)in the fire clay flue inings in mas)nry
chimneys constructed in accordance with the Stands. of the National Fire Prot= tion Association for
Chimneys, Fireplaces, Vents and Solid l uel Burning Appliances, NFPA 211, or.pplicable model
building code requirements.
Separate chimney liner parts required for installation f chimney liners are speci ted in the installation
instructions provided with the chimney liners. UnIes, specified in the individual r istings,thest chimney
liners are intended for use with gas, liquid and solid • el fired residential type : building heating
appliances specified in the Chimney Selection Chart f the National Fire Protec on Association
Standard NFPA 211.
The basic standard used to investigate products in thi. category is UL 1777, "C. . ey Liners" .

Stainless Steel Chi ney Lining ystem
Product Information
The Forever Flex stainless steel lining system is available in 3"to 16"diameters.
The lining system must be installed by a qualified chimney or venting professional.
The criteria for installation must be in conformance wilt the specifications contained in the latest versio of
the NFPA 211 (Standard for Chimneys,Fireplaces, Vents and Solid Fuel Burning Appl anees)and local 'r
state building codes, whichever has jurisdiction. Convict local building or fire oflricia s about
restrictions and installation inspection in your area. It may be neces
to obtain installing the chimney liner. Contact the local buildin authority for permit infonnatio rmits before
Product Applications
The Forever Flex lining system(A F-316Ti or HG-A 29-4C)is intended for use with heating applian.-s
burning home heating oil,natural or LP gas and solidfuels(pellet,wood and coal)ven through a
masonry chimney. Use of experimental fuels is not pe irted and voids the warranty, his lining syste is
not intended for use with Category H, Ill,or TV gas b ing appliances as defined by th National Fuel e:s
Code,NFPA.54,or other appliances that cause conden tion of corrosive acids on the 1' er of the chimn:i,
or that create positive pressures in the chimney system.
The Forever Flex system is intended for use in(1)an u lined chimney with at least(a n urinal)4"of
masonry all around,(2)a properly built masonry chicon y with cracked clay tile liners, d(3)to provid• a
properly-sized flue for a heating appliance installed in a masonry chimney that atherwis meets existing
codes.
The Forever Flex liner may also be used as a flue for a replace. The liner must be co ected to the top f-
the smoke chamber by means of a bottom plate or other means,which provides an air-ti t and drip-free
termination,

SUBJ: Notice of Violation Document received from Town of Montville dated 28 Sept
2007.

1. We have a licensed contractor planned to perform to work at 70 Pires Drive.
The contractor is licensed(#60845) and insured. The contractor is
responsible to handle all the administrative work (including filing the permit
paperwork)per our agreement.
2. No work has started on the chimney towards the re-building. The only work
performed so far is what was necessary for the contractor to provide a detailed
proposal. Removing the chimney to the roof-line was necessary for the
contractor to itemize in detailed form what was necessary. I requested a firm
fixed quote from the contractor and he could not provide this without
removing the chimney to the roofline. The removal of the chimney to the
roofline took place in early August.
3. The work to Re-build the chimney is not to start until 1st week in Nov or
Maybe Oct sometime. Regardless, the contractor is responsible to get the
Permit paperwork in place with the town hall.
4. No building materials are on-site. No Bricks, No mortar/cement material
and NO stainless steel sleeves are on-site. This is because the materials have
not been ordered.
5. In light of the current situation, I will provide the proposals which I hope are
detailed enough and I'll submit for the permit, even though I'm paying the
contractor to do this.
In summary .... No work toward the re-build of the chimney has started, the
only work that has been done was that to provide a detailed proposal for a
firm fixed price to repair the chimney. No building materials are on-site or
ever have been toward this chimney, the parts have not been ordered yet to the
best of my knowledge.
