The Town of Montville Fire Marshal conducted a final inspection of the new 8 unit apartment
building on 36 Platoz Drive. As a result of this inspection all of the requirements of the
Connecticut Fire Safety Code have been satisfied. Also let it be noted that the State Fire Marshal
issued a code waiver for fire sprinklers for the occupancy.

Dear Mr. Mandes:
I am in receipt of the As-Built Plan ("Plan) entitled "As-Built Location Survey Prepared
for John Mandes, 36 Platoz Drive, Montville, CT, Prepared by Advanced Surveys, LLC,
Dated 9/26/16" and, on 10/24/2016, I conducted a final inspection of your site to
determine compliance with your approved site plan in order to issue a Certificate of
Zoning Compliance (CZC)for you to obtain a Certificate of Occupancy(CO).
Please be advised of the following with regard to said inspection:
1. Porch gutters shall not discharge directly into and sheet flow over driveway to
town road. Prior to issuance of CZC, gutters must be connected to drainage
system as approved by Town Staff.
2. Prior to issuance of CZC, screened fencing around concrete dumpster pad shall be
installed.
3. Prior to issuance of CZC, the Applicant shall provide top soil along disturbed
area,then apply hydro seed and re-grade existing slope(max 2:1)per the Plan.
4. Prior to issuance of CZC, the Applicant shall provide a row of 8' high blue spruce
or similar plantings per the Plan.
Items #1 & 2 shall be completed prior to issuance of CZC. Due to weather constraints,
the Applicant may post a cash bond in the amount of$3,500.00 to insure final grading,
site stabilization and landscaping work per Plan is completed.

In accordance with the 2005 Connecticut Fire Safety Code Part III section 903.2.7 Group R" an
automatic sprinkler system installed in accordance with section 903.3 shall be provided
throughout all building with a Group R fire area". No sprinkler protection has been provided.

Dear Mr. Mandes,
On April 21, 2008 you applied for permits for framing interior partitions and exterior decks and
stairs at the above referenced address. This letter is to inform you that I must reject your
applications due to the following.
During my review of the files for these properties I noted that due to the age of these structures there
are no open permits for their construction. After conferring with the Office of the State Building
Inspector, I have determined that you must apply for permits for the construction of the buildings.
The interior framing and framing for the exterior decks and stairs will be included in those permits.
As part of you applications you must furnish a complete set of plans for each building, including, but
not limited to structural, mechanical, electrical, plumbing, fire suppression and alarm and energy
compliance drawings, as well as an approved site plan. You will be required to furnish two sets of
these construction documents to the Building Department and one set to the Fire Marshal.
Please be advised that the Building Department and Fire Marshals Office are willing to meet with
you and your architect and or engineer to explain these requirements and to answer any questions
you may have concerning this project.
